The anatomy of the ilio-hypogastric and ilio-inguinal nerves presents many anatomical variations that can be of great surgical importance, thus their superficial location in their distal part exposes them to risks of various lesions, lesions at the origin of postoperative neuralgia. Hence the aim of our study to try to describe an adequate surgical approach when attempting to free these nerves. The study involved the dissection of 23 inguinal regions, performed in fresh cadaveric subjects, free of pelvic trauma, and in selected patients with simple inguinal hernia and no history of inguinal region surgery. Of the 23 dissections, the iliohypogastric nerve was not found within the limits of our dissection, however, the ilioinguinal nerve was seen in 20 cases and emerged an average of 4.35 cm from the anterocranial iliac spine, an average of 1.93 cm from the pubic spine, and an average of 9.09 cm from the navel.
