Periapical radiographs are commonly employed to evaluate the root canal morphology. However, it provides a two-dimensional image of a three-dimensional object and hence there is always a chance of missing any important structure present in the third dimension. Cone beam computed tomography (CBCT) provides an accurate three-dimensional image of the entire tooth and is considered to be the most accurate method for assessment of the root and canal morphology. Mandibular first molar is the first permanent tooth that erupts into the oral cavity and is frequently indicated for root treatment because of early exposure to caries. Therefore, its morphology has received considerable attention in the dental literature. The write up is about the root and canal morphology of mandibular first molars in a sample of Pakistani population using CBCT images and to compare it with the data published in international literature. The success of root canal treatment largely depends on the dentist's understanding of canal morphology, hence the present work will improve the quality of root canal treatment in mandibular first permanent molar by reducing its failure and to avoid unnecessary economic burden.
